Mental Health Parity Act. The plan will not have a separate calendar year maximum for outpatient mental health benefits. Services provided by a physician or other provider shall be subject to the following: visits 1-10 will be covered at one hundred percent (100%) of reasonable and customary. Visits 11-36 will be covered at eighty percent (80%) of reasonable and customary.
