Common use of Life Insurance and Long-Term Disability Clause in Contracts

Life Insurance and Long-Term Disability. During the term of this Agreement, nurses will participate in the life insurance and long-term disability plan as in accordance with the provisions of the SHS plan. During the term of this Agreement, nurses may also participate in the voluntary insurance plans in accordance with the provisions in the SHS plan.

Life Insurance and Long-Term Disability. During the term of this Agreement, 2 nurses will participate in the life insurance and long-term disability plan as in 3 accordance with the provisions of the SHS plan. During the term of this 4 Agreement, nurses may also participate in the voluntary insurance plans in 5 accordance with the provisions in the SHS plan.

Life Insurance and Long-Term Disability. During the term of this Agreement, nurses 18 will participate in the life insurance and long-term disability plan as in accordance 19 with the provisions of the SHS plan. During the term of this Agreement, nurses may 20 also participate in the voluntary insurance plans in accordance with the provisions in 21 the SHS plan.

Life Insurance and Long-Term Disability. During the term of this 5 Agreement, nurses will participate in the life insurance and long-term disability plan as 6 in accordance with the provisions of the SHS plan. During the term of this Agreement, 7 nurses may also participate in the voluntary insurance plans in accordance with the 8 provisions in the SHS plan.

During the term of this Agreement, 15 nurses will participate in the life insurance and long-term disability plan as in 16 accordance with the provisions of the SHS plan. During the term of this 17 Agreement, nurses may also participate in the voluntary insurance plans in 18 accordance with the provisions in the SHS plan.

During the term of this Agreement, 31 nurses will participate in the life insurance and long-term disability plan as in 32 accordance with the provisions of the SHS plan. During the term of this 22 Agreement, nurses may also participate in the voluntary insurance plans in 23 accordance with the provisions in the SHS plan.
