Leukemia Payroll Deduction. The Employer agrees to provide prior to January 1st, 2022 a payroll deduction for the U.F.C.W. Leukemia Research Fund for all employees who wish to contribute. In addition to the above, the employer shall present the enrolment form at orientation meeting to all new employee and when completed, such form (s) and deduction if any shall be sent to the Union office with a detailed list of names, Social Insurance Number and amount deducted, remit same by cheque payable to the UFCW Leukemia Research Fund on or before the fifteenth (15th) day of the following month the money was deducted. The total amount deducted in the calendar year will be provided by the employer on or before February 28th of each year and noted by the employer on the employee’s T4 slip.
