HYDROXYCHLOROQUINE. Hydroxychloroquine is an anti-malarial drug which can also be used as a disease modifying agent in rheumatoid arthritis, discoid or systemic lupus erythematosus and other photosensitive dermatological conditions. This medication is generally well tolerated. There is a rare but important risk of retinal toxicity. Dosing Regimen To reduce the risk of retinal toxicity, the recommended maximum dosing is 5mg/kg/day based on actual body weight, practically, this will be 200mg once daily or 200mg twice daily. In CKD stage 4-5 a dose reduction of 25 -50% is recommended. Schedule of Monitoring Pre-treatment: Baseline FBC, U&E and LFT Weight must be recorded to allow accurate dosing Baseline formal ophthalmic examination ideally including optical coherence topography (OCT) within one year of commencing drug should be reserved for patients with renal impairment, concomitant tamoxifen therapy or patients with pre-existing retinal and macular problems. This initial screening will be arranged by the consultant initiating the medication. Monitoring: No routine blood monitoring is required for hydroxychloroquine. It is recommended that patients have an annual eye assessment, ideally including OCT, if the medication is continued for more than five years. The OCT scans will be arranged by the secondary care team therefore there is no monitoring required by the primary care physician.

  • Asbestos Any material that contains more than one percent asbestos and is friable or is releasing asbestos fibers into the air above current action levels established by the United States Occupational Safety and Health Administration.

  • Influenza Vaccine Upon recommendation of the Medical Officer of Health, all employees shall be required, on an annual basis to be vaccinated and or to take antiviral medication for influenza. If the costs of such medication are not covered by some other sources, the Employer will pay the cost for such medication. If the employee fails to take the required medication, she may be placed on an unpaid leave of absence during any influenza outbreak in the home until such time as the employee has been cleared by the public health or the Employer to return to the work environment. The only exception to this would be employees for whom taking the medication will result in the employee being physically ill to the extent that she cannot attend work. Upon written direction from the employee’s physician of such medical condition in consultation with the Employer’s physician, (if requested), the employee will be permitted to access their sick bank, if any, during any outbreak period. If there is a dispute between the physicians, the employee will be placed on unpaid leave. If the employee gets sick as a reaction to the drug and applies for WSIB the Employer will not oppose the application. If an employee is pregnant and her physician believes the pregnancy could be in jeopardy as a result of the influenza inoculation and/or the antiviral medication she shall be eligible for sick leave in circumstances where she is not allowed to attend at work as a result of an outbreak. This clause shall be interpreted in a manner consistent with the Ontario Human Rights Code.

  • Hepatitis B Vaccine Where the Hospital identifies high risk areas where employees are exposed to Hepatitis B, the Hospital will provide, at no cost to the employees, a Hepatitis B vaccine.

  • Dangerous Goods, Special Wastes, Pesticides and Harmful Substances Where employees are required to work with or are exposed to any dangerous good, special waste, pesticide or harmful substance, the Employer shall ensure that the employees are adequately trained in the identification, safe handling, use, storage, and/or disposal of same.

  • TOXIC SUBSTANCES Each Contractor furnishing a toxic substance as defined by Section 875 of the Labor Law, shall provide such Authorized User with not less than two copies of a material safety data sheet, which sheet shall include for each such substance the information outlined in Section 876 of the Labor Law. Before any chemical product is used or applied on or in any building, a copy of the product label and Material Safety Data Sheet must be provided to and approved by the Authorized User agency representative.
