Final Performance and Financial Report (required. This report is due 90 days after the end of the period of performance. CDC programs must indicate that this report should not exceed 40 pages. This report covers the entire period of performance and can include information previously reported in APRs. At a minimum, this report must include the following:  Performance Measures – Recipients must report final performance data for all process and outcome performance measures.  Evaluation Results – Recipients must report final evaluation results for the period of performance for any evaluations conducted.  Impact/Results/Success Stories – Recipients must use their performance measure results and their evaluation findings to describe the effects or results of the work completed over the project period, and can include some success stories.  A final Data Management Plan that includes the location of the data collected during the funded period, for example, repository name and link data set(s)  Additional forms as described in the Notice of Award (e.g., Equipment Inventory Report, Final Invention Statement). The Final Performance Report should include information to fulfill any specific reporting requirements in the assistance award, a summary statement of progress toward the achievement of the originally stated aims, a list of the results (positive or negative) considered significant, and a list of publications resulting from the project, with plans, if any, for further publications. An original and two copies of the report are required.
