Establishment of Service. 6.1 After receiving certification as a local exchange carrier from the applicable regulatory agency, TWTC will provide the appropriate AT&T Advisory team manager the necessary documentation to enable AT&T to establish accounts for resold services ("master account"). TWTC is required to provide the following before a master account is established: blanket letter of authorization, misdirected number form, proof of PSC/PUC certification, the Application for Master Account, an Operating Company Number ("OCN") assigned by the National Exchange Carriers Association ("NECA") and a deposit and tax exemption certificate, if applicable.
