Density/Intensity. This element describes the permitted density, building sizes, lot sizes and general site development potential. These standards serve as regional limits. Exact densities/intensities shall be determined by local zoning ordinances, and it is not assured that the stated density/intensity standards within the Future Land Use Matrix are an entitlement to landowners. Standards not addressed within the Matrix are at the discretion of each municipality.

In a Provider’s Office/In Your Home This plan covers individual psychotherapy, group psychotherapy, and family therapy when rendered by: • Psychiatrists; • Licensed Clinical Psychologists; • Licensed Independent Clinical Social Workers; • Advance Practice Registered Nurses (Clinical Nurse Specialists/Nurse Practitioners- Behavioral Health); • Licensed Mental Health Counselors; and • Licensed Marriage and Family Therapists. Psychological Testing This plan covers psychological testing as a behavioral health benefit when rendered by: • neuropsychologists; • psychologists; or • pediatric neurodevelopmental specialists. This plan covers neuropsychological testing as described in the Tests, Labs and Imaging section.

  • OPTICAL ‌ The Employer agrees to provide Optical coverage for active full- time employees in the amount of one hundred twenty-five dollars ($125.00) every twenty-four (24) month period, for the cost of frames, lenses and the fitting of prescription glasses when recommended by a doctor or optometrist. Sunglasses or glasses for cosmetic purposes are not included nor is the cost of eye examinations. While this Appendix is intended to provide an overview of all benefit coverages the insurer’s plan documents will ultimately govern the administration of these benefits.
