Commonwealth Connections Academy ONLY: ACCESS Program. Connections reserves the right to claim medical reimbursement through the Pennsylvania Department of Education/Department of Public Welfare – based ACCESS Program for an student that the Contractor and Contractor's staff service through this Agreement. It is understood that the Contractor and Contractor's staff will provide the required documentation necessary in order for Connections to claim this reimbursement, using whatever forms, procedures, and training Connections prescribes, for any period of the program year. All applicable ACCESS paperwork is to be completed and submitted to Connections by the 10th of each month following the date of service delivery. If the ACCESS paperwork is not submitted by the end of the month following the month of service, Connections will not be responsible for payment on the Contractor’s invoice until such time as the document is received.]
