Certificates of Insurance/Notices. CONSULTANT shall provide a Certificate or Certificates of Insurance, in a form satisfactory to the Commissioner, before commencing any work under this contract. Certificates or transmittal correspondence shall reference the NYSDOT Contract D Number. Certificates shall be mailed to the: New York State Department of Transportation Contract Management, 0xx Xxxxx 00 Xxxx Xx.
