Athletic Training Services. During the athletic events at which Athletic Trainers are in attendance, they shall attend to any injuries sustained by School students as a result of the student's participation in the athletic event. Services provided by Athletic Trainers may include, without limitation: (1) performing physicals for student athletes as necessary, (2) conducting an initial assessment of an athlete's injury or illness and formulating an impression of the injury or illness in order to provide emergency care and referral to a physician for definitive diagnosis and treatment, if necessary; (3) administering first aid and emergency care for acute athletic injuries and illnesses; and (4) providing health care information to and counseling athletes while at the athletic event. Before athletic events, Athletic Trainers may carry out injury prevention measures to minimize the risk of injury during play. In addition, Athletic Trainers will provide on- site injury checks each week during competitive seasons, pre-participation physicals to student athletes at each school (mass participation format only), and will offer ImPACT testing for all high school student athletes in district after district approval (cost paid by student athlete or school). All examinations and treatment of student athletes will be performed in the presence of the parent, coach, another student athlete, or school designee. All services provided by Athletic Trainers will be in accordance with state and federal law, Board of Education policies and administrative procedures, and MSHSAA standards and guidelines with respect to the provision of such services, specifically including the assessment and treatment of concussions.
Athletic Training Services. The Athletic Trainer providing AT Services to the School will have (or will be in as of the Commencement Date diligently pursuing) received certification by the Board of Certification, Inc. and licensure from the Commonwealth of Pennsylvania. In addition, the Athletic Trainer will have completed (or will be in the process of completing) the training requirements of the Child Protective Services Law and a PA Criminal Background Check, PA Child Abuse History and PA FBI Clearance. Copies of all documentation will be forwarded to the School upon request.
Athletic Training Services. During the School Year, the ATs will attend practices and games of the School District’s sports teams (“School Events”), except where the School District has scheduled multiple events at the same time, one or more of which is scheduled to occur outside the School District property. In the event of conflict, the ATs and the Athletic Director will determine which events are to be covered. • Upon request, ATs will provide Services for additional events that are not School Events, such as playoff games and league games. School District shall notify LGMG no less than 48 hours in advance of the non-School Event when requesting Athletic Trainer Services. • ATs will see out-of-season student athletes as time permits, while granting first priority to in-season student athletes. • ATs will advise the School District as to the supplies and training equipment required for the School District’s athletic training programs. • ATs will assist the School District’s faculty and coaching staff in the design and implementation of a student athletic training aide program. • ATs will provide continuing education programs to the School District’s coaching staff. • ATs will attend preseason coaches’ meetings, and upon request, PTO and school board meetings. • ATs will coordinate and provide injury follow up and evaluations for the School District’s team physician. • ATs will monitor athletic injuries and develop injury prevention training and programs with the advice and consent of School District’s team physician. • School District is responsible for contracting with Emergency Medical Services (“EMS Personnel”), if so desired. Therefore, services under this agreement shall not include emergency medical services, unless such treatment is needed prior to arrival of EMS Personnel or to assist EMS Personnel as needed. • LGMG will work to provide priority access for School District student-athletes to LGH physical therapy at all Lancaster General Health locations. • LGMG shall provide information and treatment protocols to School District and student-athletes regarding concussions and other common sports injuries, as well as guidelines addressing post-injury return-to-play. • LGMG shall, in coordination with athletic department provide 1 mass sports pre-participation physical over the summer in preparation for the upcoming school year on-site at a designated school district facility. Athletic Training Services. Contractor agrees to furnish those athletic training services as set forth on Exhibit A attached hereto (the “Athletic Training Services”) during such times and at those events set forth on Exhibit B attached hereto. In the event Contractor’s attendance would be required at more than one (1) event as set forth on Exhibit B as a result of multiple events being scheduled for the same time, the Athletic Director for the School will give Contractor reasonable prior notice as to whether Contractor shall divide its time between events or whether the Contractor shall only attend one (1) of the events. Contractor represents that its Athletic Training Services shall be provided in a first-class high quality and professional manner and that it has the background and expertise and personnel necessary to provide the Athletic Training Services for the School.
Athletic Training Services. MNPG shall provide up to two (2) licensed athletic trainers (the “Athletic Trainers”) for the period beginning on the first day of fall sports heat acclimation practices and shall continue to provide such Athletic Trainers until the conclusion of spring sports competition. As long as MNPG is exercising reasonable efforts to hire such Athletic Trainers in a timely manner, MNPG shall not be liable for any delays in performance related to its inability to hire such Athletic Trainers. Athletic training student supervision; and 9. Care consistent with the protocols determined in cooperation with the Physician (as defined below).

Melbourne Archdiocese Catholic Schools Ltd (MACS) governs the operation of MACS schools and owns, governs and operates the School. 1.2 Parents and guardians, as the first educators of their children, enter into a partnership with the Catholic school to promote and support their child’s education. Parents and guardians must assume a responsibility for maintaining this partnership by supporting the school in the provision of education to their children within the scope of School's registration and furthering the spiritual and academic life of their children.

  • Counseling Services Your first appointment (or more, in some cases) will involve an assessment of your needs. By the end of the assessment your counselor will be able to offer you some first impressions of what your work may include and recommendations for getting help. One of the recommendations may be psychotherapy. If so, ABO may or may not be able to provide you with psychotherapy, depending on your overall needs. If psychotherapy or any other recommendations suggested by your counselor include things that ABO cannot provide, you will be given suggestions of where you might receive those services. Wherever you choose to obtain treatment, you should evaluate the information from your initial assessment along with your own opinions of what sort of treatment you are willing to do and whether you feel comfortable working with the treating clinician. Therapy involves a large commitment of time, energy, and often money, so you should be very careful about the counselor you select. If you have questions about the procedures used or conclusions made by your counselor at ABO, please discuss them whenever they arise. If your doubts persist, your counselor will be happy to help you set up a meeting with another mental health professional for a second opinion. Psychotherapy is not easily described in general statements. It varies depending on the particular problems you are experiencing, the therapeutic methods used by your counselor, and the personalities of the counselor and client. There are many different methods counselors may use to deal with the problems that you hope to address. Psychotherapy is not like a medical doctor visit. Instead, it calls for a very active effort on your part. In order for the therapy to be most successful, you will have to work on things that are discussed both during your sessions and on your own. Psychotherapy can have benefits and risks. Since therapy often involves discussing unpleasant aspects of your life, you may experience uncomfortable feelings like sadness, guilt, anger, shame, frustration, loneliness, and helplessness. The changes you make in therapy may also affect your relationships in unexpected ways. Psychotherapy has also been shown to have many benefits. Therapy often leads to better relationships, solutions to specific problems, and significant reductions in feelings of distress. But there are no guarantees of what you will experience. The initial assessment will last from one to four sessions. During this time, you and your counselor can both decide if she/he is the best person to provide the services you need in order to meet your treatment goals. If you choose to begin psychotherapy, your counselor will usually schedule one meeting at a time you each agree on. Once an appointment is scheduled, you will be expected to attend unless you provide advance notice of cancellation. If you need to cancel an appointment, it is your responsibility to contact us to cancel. Fees will be collected when services are rendered.*In addition to therapy services fees may include assessment administration, scoring, and reporting; creating reports; creating copies of records on request; and consulting with other professionals at the client’s request. If you still must change or cancel your counseling appointment, please know: • Cancellations must be made during business hours. • Cancellations must be made within 48 hours of session time if using our online scheduling program. • We cannot accommodate cancellations made via email as we do not monitor email on a regular basis. • We may not accommodate cancellations made after hours or on holidays to our main number as we cannot check voice mail regularly. Email, and text messaging are not secure mediums in terms of privacy and confidentiality so our policy regarding, electronic communication, and cell phone use includes the following: • We do not provide therapy/counseling via email or text messaging. • Text messaging and email will be used for administrative tasks only (as defined above). • Therapists may not acknowledge or return emails or text messages that are not administrative. This includes emergency texts and emails. • If your therapist leaves for an extended period of time you will be given the information for another licensed therapist with whom you may schedule if you need an appointment during your therapist’s absence.
