Appointment of Residents. The Hospital, through its designee, shall be responsible for annually determining and thereafter appointing the Resident to the appropriate postgraduate year of the Residency Program. Reappointment of Resident shall be determined by each Program’s Policies and the guidelines of that Program’s accrediting body, specialty and subspecialty board.
Appointment of Residents. REPRESENTATIVE (OPTIONAL): The Resident hereby designates (name a person): to be the “Resident’s Representative” as defined by Section 45/1-123 of the Nursing Home Care Act (210 ILCS 45/1-123). The Resident shall provide to the Facility a copy of any written agreement between the Resident and the Resident’s Representative which authorizes the Resident’s Representative to inspect and copy the Resident’s records and/or authorizes the Resident’s Representative to execute this Contract on behalf of the Resident.
