Common use of Application for PTO Clause in Contracts

Application for PTO. 10 19.5.1 PTO leave of at least two (2) consecutive weeks will be established on a 11 first-come-first serve basis by date of application in the designated 12 vacation book or the appropriate request form. In the event two or more 13 nurses request the same time and make a request on the same calendar 14 date, the most senior nurse will be granted the PTO time requested. A 1 nurse who exercises a seniority preference for scheduling such time off 2 may not again exercise a seniority preference during the next two (2) 3 years. Requests for earned PTO of at least two (2) consecutive weeks 4 should be submitted at least six (6) weeks prior to the schedule being 5 posted. Requests for PTO should not be made more than nine (9) months 6 in advance, unless special circumstances exist where travel or other 7 arrangements must be made in advance.
