  • Unless otherwise specified, it applies to all Covered Benefits except the Preventive Health Care Services Benefit.

  • The services required by the Affordable Care Act and their frequency and time frames are stated in the Preventive Health Care Services Schedule (“Schedule”).

  • These services will be covered as shown in the Preventive Health Care Services, and/or the Preventive Contraceptive Methods and Counseling for Women sections of this COC.

  • Compounded drugs containing any product that is excluded by us will not be covered, including dosages and route of administration that have not been approved by the FDA.Prescription Drugs covered as Preventive Health Care Services.

  • Please see the Preventive Health Care Services section for coverage of prescription drugs, including certain insulin and other glucose lowering agents, on PIC’s Preventive Drug List.

  • Preventive Health Care Services PIC covers preventive health care services required under the Affordable Care Act that you receive during the calendar year.

  • Home health care services means medical and nonmedical services, provided to ill, disabled or infirm persons in their residences. Such services may include homemaker services, assistance with activities of daily living and respite care services.

  • PREVENTIVE CARE SERVICES means covered healthcare services performed to prevent the occurrence of disease as defined by the Affordable Care Act (ACA). See Preventive Care and Early Detection Services in Section 3. PRIMARY CARE PROVIDER (PCP) means, for the purpose of this plan, professional providers that are family practitioners, internists, and pediatricians. For the purpose of this plan, gynecologists, obstetricians, nurse practitioners, and physician assistants may be credentialed as PCPs. To find a PCP or check that your provider is a PCP, please use the “Find a Doctor” tool on our website or call Customer Service.

  • Health care services means services for the diagnosis, prevention, treatment, cure, or relief of a health condition, illness, injury, or disease.

  • Basic health care services means in and out-of-area emergency services, inpatient hospital and

  • Child care services means the range of activities and programs provided by a certificate holder to an enrolled child, including personal care, supervision, education, guidance, and transportation.

  • Home Health Care means the continual care and treatment of an individual if:

  • Indian Health Care Provider means a health care program operated by the Indian Health Service (IHS) or by an Indian Tribe, Tribal Organization, or Urban Indian Organization (otherwise known as an I/T/U) as those terms are defined in § 4 of the Indian Health Care Improvement Act (25 USC § 1603). Indian Health Care Provider includes a 638 Facility and provision of Indian Health Service Contract Health Services (IHS CHS).

  • Health care plan means any contract, policy or other arrangement for benefits or services for medical or dental care or treatment under:

  • Health and Human Services Commission or “HHSC” means the administrative agency established under Chapter 531, Texas Government Code, or its designee.

  • Day-care services means care of a child for a portion of the day which is less

  • Licensed health care provider means a physician, physician assistant, chiropractor, advanced registered nurse practitioner, nurse, physical therapist, or athletic trainer licensed by a board.

  • Health care worker means a person other than a health care professional who provides medical, dental, or other health-related care or treatment under the direction of a health care professional with the authority to direct that individual's activities, including medical technicians, medical assistants, dental assistants, orderlies, aides, and individuals acting in similar capacities.

  • Preventive Care means certain Preventive Care services.
