Health Passport definition

Health Passport means a summary of health information for each DCFS Youth in Care that contains the youth’s health history, present health care and medical conditions, if any, and available health information about the youth necessary for the youth’s proper care.
Health Passport means an electronic health record used to document information regarding medical services provided to an FC Covered Person.
