Once this agreement is approved, it may be necessary to complete an additional form(s), known as the FMS Vendorization Sheet, if not already on file with VA. When submitting please attach a scanned copy of the signed form, and an addendum of additional covered campuses if applicable. (VA cannot process applications received without a signature).
