Common use of Services Excluded Clause in Contracts

Services Excluded. Membership in the PRACTICE does not include immunizations, laboratory tests, medical devices, emergency room medical services, specialist care, hospital care, radiology, or other specialty health care needs. If the Provider feels a healthcare needis outside of thescope of his/her training, referral to aspecialist will be warranted.
