Self-directed Care. The Contractor will support the Enrollee in actively participating in the development of the Enrollee Care Plan. The Contractor will also encourage Providers to support Enrollees in directing their own care and Enrollee Care Plan development. This will include giving PCPs a copy of the Enrollee Care Plan.
