Common use of Requesting Catastrophic Leave Clause in Contracts

Requesting Catastrophic Leave. An employee who is eligible for catastrophic leave under subsection 3 of this Section may request that a specified number of hours of leave be transferred from the catastrophic leave account to his/her account. The request must be submitted in writing to the Human Resources Department. The maximum number of hours that may be requested is two hundred forty (240) hours per catastrophe. The request must include: (i) the employee's name, title and classification; and (ii) a statement from the employee’s attending provider substantiating the catastrophe and the period of incapacity resulting therefrom. The cost of all substantiating medical information submitted by the employee shall be borne by the employee.
