Reporting of Concerns about Harassment and/or Discrimination. Nurses are encouraged to utilize the Medical Center's internal complaint reporting processes for reporting concerns regarding harassment and/or discrimination.

Reporting of Concerns. The Medical Center supports and encourages 2 nurses to raise and document any staffing concerns, without fear of 3 retaliation. For specific staffing concerns, a nurse may use the Staffing 4 Request and Documentation Form (SRDF). Nurses will submit completed 5 forms, to their manager, and the co-chairs of the House-Wide Staffing 6 Committee. A nurse requested by the Medical Center to review an SRDF 7 report they have submitted may have an Association representative present 8 with them during this review. During a review of a submitted SRDF, the 9 Medical Center may request additional information from the submitting nurse.
