Program Performance Measures. Grantee performance shall be determined using the following measures: 1. Total formal referrals in each year of the grant period; 2. Average daily population of juveniles under deferred prosecution and probation supervision in each year of the grant period; 3. Number of placements in secure and non-secure residential post-adjudication facilities in each year of the grant period; 4. Percentage of juveniles under deferred prosecution and probation supervision participating in a community- based program in each year of the grant period; 5. Number of juveniles committed to the Department in each year of the grant period; 6. Number of juveniles certified as adults in each year of the grant period; 7. Percentage of Department-eligible offenders committed to the Department; 8. One, two, and three year re-referral/re-arrest and incarceration recidivism rates for juveniles beginning supervision, beginning a program, and leaving a post-adjudication residential facility; 9. Total number of juveniles provided mental health services during the contract period; 10. Total number of mental health assessments or psychological evaluations completed; 11. Total number of mental health treatment hours provided; 12. Number of juveniles offenders served by State Aid Grant Commitment Diversion funding; 13. Percent of juveniles served completing Commitment Diversion programs or placements; 14. Number of juveniles served by State Aid Grant Commitment Diversion funding committed to TJJD by the juvenile probation department during the grant period; 15. One, two, and three year recidivism rates for all juveniles served by State Aid Grant Commitment Diversion funding in programs and post-adjudication placements.
Program Performance Measures. 1. Contractor shall administer the same district and state-required assessments in the same manner and timeframe and follow, at a minimum, the same promotion and graduation requirements as all MPS schools, as described in the MPS Administrative Policy 7.37, Graduation Requirements, attached hereto and incorporated herein by reference as Appendix D, or as approved in writing by the Director of Contracted School Services. Contractor explicitly agrees to comply with the assessment and accountability provisions of P.L. 114-95, Every Student Succeeds Act of 2015 (ESSA) as they are made applicable to MPS and Contractor. 2. In addition to the requirements set forth in paragraph I.C.1. above, Contractor may choose to define and implement up to two local performance goals as well as up to two local assessments to measure student achievement and gauge progress against locally established goals. Performance goals and assessments must be pre-approved by CSS prior to the execution of this Contract. Contractor may report all such data regarding local performance measures in its annual pupil academic achievement report. Contractor’s decision to implement unique standards and assessment shall not affect Contractor’s obligation to comply with the standards adopted by the State of Wisconsin and federal, state and district assessment and accountability measures. 3. All pupils attending the Educational Program under this Contract shall be included in the assessment process, with reasonable accommodations for pupils being made if necessary. Contractor shall include children with disabilities and English Language Learners (ELL) in all federal, state, district, and local educational assessments, with appropriate modifications where necessary, or alternative assessments for those children who cannot participate in federal, statewide, district-wide or local educational agency-wide assessments. 4. The Board shall pay all costs associated with district-required assessments administered to pupils attending the Educational Program under this Contract. Contractor shall participate in the Wisconsin Alternate Accountability process designed by the Wisconsin Department of Public Instruction if the school does not meet criteria for state level school report cards. 5. In order to be considered in compliance with the performance criteria, the Contractor must submit to Health Care Agency a request for reimbursement which demonstrates that Contractor has expended funds and met their proposed accomplishment goals at the required levels, unless exempted in writing by the Director. Failure to achieve performance may cause any remaining balance in this Contract to be reclaimed by County and will negatively affect future funding to Contractor. CONTRACTOR will document and report this data to COUNTY as described in Section C .3. “Reporting Requirements”.   • Ongoing Performance Measures The Department intends to use performance-reporting tools in order to measure the performance of Contractor(s). These tools will include the Contractor Performance Survey (Exhibit G), to be completed by Customers on a quarterly basis. Such measures will allow the Department to better track Vendor performance through the term of the Contract(s) and ensure that Contractor(s) consistently provide quality services to the State and its Customers. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MFMP or on the Department's website).

  • Performance Measure Grantee will adhere to the performance measures requirements documented in

  • Performance Measures The System Agency will monitor the Grantee’s performance of the requirements in Attachment A and compliance with the Contract’s terms and conditions.

  • Performance Measurement The Uniform Guidance requires completion of OMB-approved standard information collection forms (the PPR). The form focuses on outcomes, as related to the Federal Award Performance Goals that awarding Federal agencies are required to detail in the Awards.

  • Performance Metrics The Influencer shall aim for a minimum engagement rate of [SPECIFY PERCENTAGE, e.g., 3%] on all posts associated with the Campaign. Engagement rate is calculated as the sum of likes, comments, shares, and other interactions divided by the total number of followers at the time of posting. The Influencer agrees to achieve a minimum reach of [SPECIFY NUMBER] unique viewers per post, or a cumulative reach of [SPECIFY NUMBER] across the Campaign. Impressions data will be provided through the Influencer’s analytics tools and verified by the Company when requested. For posts incorporating a call-to-action, such as links to the Company’s website or landing page, the Influencer will target a CTR of at least [SPECIFY PERCENTAGE, e.g., 2%]. CTR is measured as the ratio of clicks to impressions, based on data from tracking links provided by the Company. The Influencer may be expected to drive specific actions (e.g., sales, sign-ups, downloads) using unique tracking codes or referral links. Specific conversion targets will be detailed between the Parties. The Influencer shall submit performance reports on a [WEEKLY/BI-WEEKLY/MONTHLY] basis. These reports must include detailed metrics for each published post, such as: number of likes, comments, shares, and other engagement interactions; reach and impressions per post; click-through data and referral link activity; and conversion data (if applicable). Within [NUMBER] days following the end of the Campaign, the Influencer shall provide a comprehensive post-campaign report summarizing overall performance against all agreed KPIs, including supporting documentation (e.g., screenshots, analytics dashboard exports). The Influencer agrees to provide access to analytics platforms or third-party verification tools to authenticate the reported data, if requested by the Company. The Parties agree to conduct a review of the performance metrics within the first [NUMBER] days of the Campaign to ensure the targets remain realistic and reflective of current market conditions. Adjustments may be made in writing if necessary. If the Influencer consistently fails to meet the established KPIs without valid justification, the Parties shall meet in good faith to discuss potential remedies, which may include adjustments to the compensation structure or additional promotional support, as mutually agreed upon. The Company may specify certain analytics tools or platforms for measuring and reporting performance metrics. The Influencer shall utilize these specified tools where applicable to ensure consistency and transparency in data reporting. In instances where independent verification of performance data is required, the Influencer agrees to cooperate with third-party verification services designated by the Company to validate the metrics reported.
