Please fill out the following information and return the form to the school prior to the first CLUB CARE visit. The older sibling must make sure to “Look the Teacher in the Eyes” and confirm that they are retrieving their sibling. Parents, please don’t “visit with friends” when you could be picking up your children. Pick up at the earliest possible opportunity. Thanks.
