Notification by the Claims Reviewer or the Secondary Claims Reviewer as the case may be, to the MCC Settlement Trust that a Participating Claimant is eligible for payment and thus a Qualified Claimant shall be made in writing in accordance with the Qualification Notice Form attached as Exhibit F to the Master Settlement Agreement, and in accordance with the relevant sections of Articles V and VI of the Master Settlement Agreement, shall contain the following information:
