Common use of Life, Accidental Death and Dismemberment Insurance Clause in Contracts

Life, Accidental Death and Dismemberment Insurance. The Company will provide eligible employees coverage equaling one (1) times the employee’s base annual wage. Participating employees must provide proof of insurability as required. Employees may elect additional life insurance at their own expense with proof of good health. Employees may elect dependent life coverage at their own expense with proof of good health.
