IMMUNIZATION SHOTS. The School Committee shall make available, when recommended by the State Department of Health, immunization shots for all teachers and their families at the minimal cost of actual per capita expense to the School District and shall be on a voluntary basis.
IMMUNIZATION SHOTS. The City will continue to provide tetanus and typhoid inoculations to employees whose jobs require such shots according to past practice. The City is willing to expand the type of immunization shots given but must maintain control over which shots and which category of employee is to receive them. The City also agrees to allow Sanitation and Water Department employees inoculations for Tetanus and Typhoid at the City’s clinic.
IMMUNIZATION SHOTS. The employer shall pay the cost of inoculation for employees against Hepatitis B, Tetanus, Diphtheria, Mumps Measles and Rubella, or any other inoculations required for employees.
IMMUNIZATION SHOTS. A. The Board shall provide immunization shots to each teacher annually, without cost to the staff member. Notification of such shots shall be sent to each teacher at least two (2) days prior to the administering of the shots, stating the times and locations of their distribution. B. Participation in this program shall be entirely voluntary *(Cold shots, Flu shots, and any other necessitated by a prevailing epidemic).
IMMUNIZATION SHOTS. Upon request, the Town of Southwick will provide each officer the following immunization shots: Tetanus, Hepatitis, TB Titer, Hep B Titer, and Tuberculosis. 1. Representatives of the Employer and the Union Negotiating Committee may meet from time to time for the purpose of reviewing the administration of the agreement with particular emphasis upon any mandatory statute of the Commonwealth of Massachusetts enacted prior to or after the date of this agreement as well as any permissive legislation which may be enacted and which may affect this agreement or the operation of the Police Department. 2. Each party may make a written request for a meeting which shall be scheduled within thirty (30) days. Any such request shall state the purpose of the meeting. Either party may submit to the other, at least ten (10) days prior to the meeting, an agenda covering matters they wish to discuss.
IMMUNIZATION SHOTS. 25 The district agrees to pay all expenses for inoculation or immunization shots for the employee 26 and for the members of the employee's family residing in his/her household when such shots 27 become necessary as determined by a physician as a result of said employees exposure to 28 contagious disease where said employee has been exposed to said disease in the line of duty. 29 The District shall provide to those employees who voluntarily request it, vaccinations against 30 all types of Hepatitis, Flu Vaccines, and any diseases occupationally acquired, with the 31 District paying the full cost. Employees not requesting said vaccinations shall sign a waiver 32 each year.
IMMUNIZATION SHOTS. The Town agrees to pay all expenses for inoculation or immunization shots for the employee and for the members of the employee's family residing in his/her household when such shots become necessary as determined by a physician as a result of said employees’ exposure to contagious disease in the line of duty. The Town shall provide to any employee, upon request, vaccinations against all types of Hepatitis, Flu, and any diseases occupationally acquired, with the Town paying the full cost.

  • Immunization The following immunizations are recommended by the BSA. Tetanus immunization is required and must have been received within the last 10 years. If you had the disease, check the disease column and list the date. If immunized, check yes and provide the year received.

  • Searchable Whois Notwithstanding anything else in this Agreement, Registry Operator must offer a searchable Whois service compliant with the requirements described in Section 1.10 of Specification 4 of this Agreement. Registry Operator must make available the services only to authenticated users after they logged in by supplying proper credentials (i.e., user name and password). Registry Operator must issue such credentials exclusively to eligible users and institutions that supply sufficient proof of their legitimate interest in this feature (e.g., law enforcement agencies).

  • Intake Intake begins when you, or someone on your behalf, contacts the LIFE Provider or the Independent Enrollment Broker expressing interest in services. If it appears from this first conversation that you are potentially eligible, a LIFE Provider and Independent Enrollment Broker representative will contact you to explain the program, obtain further information about you, and to schedule in person or tele-visits. During these visits: • You will learn how the LIFE Program works, the services LIFE offers, and the answers to any questions you may have about LIFE. • The LIFE Provider and/or Independent Enrollment Broker will explain that if you enroll, you must agree that all of your healthcare services will be provided and/or coordinated by LIFE, including primary care and specialist physician services (other than emergency services). • The LIFE Provider will have you sign a release allowing the LIFE Provider to obtain your past medical records so the LIFE health team can fully assess your health conditions. You will be encouraged to visit the LIFE Center to see what it is like. If you are interested in enrolling, a LIFE Provider representative and the Independent Enrollment Broker will assist you with the enrollment process. You should be prepared to participate in phone calls and/or visits with both the LIFE Provider and Independent Enrollment Broker in order to complete your enrollment process.

  • System Logging The system must maintain an automated audit trail which can 20 identify the user or system process which initiates a request for PHI COUNTY discloses to 21 CONTRACTOR or CONTRACTOR creates, receives, maintains, or transmits on behalf of COUNTY, 22 or which alters such PHI. The audit trail must be date and time stamped, must log both successful and 23 failed accesses, must be read only, and must be restricted to authorized users. If such PHI is stored in a 24 database, database logging functionality must be enabled. Audit trail data must be archived for at least 3 25 years after occurrence.
