Immunizations. (except for those preventive immunizations for routine use in children, adolescents, and adults that have in effect a recommendation from the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention);
Immunizations. Coverage is provided for immunizations for routine use in children, adolescents, and adults that have in effect a recommendation from the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention with respect to the individual involved. Immunizations required solely for travel or work are not covered. A recommendation from the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention is considered to be:
1. In effect after it has been adopted by the Director of the Centers for Disease Control and Prevention; and
2. For routine use if it is listed on the immunization schedules of the Centers for Disease Control and Prevention.
Immunizations. Employees shall receive, upon request, flu shots as prescribed by medical standards paid for by the employer using the most cost effective system of delivery in the community, or at the employee’s option at no cost to the employee through the employer sponsored health plan. The employer will offer treatment at no cost to the employee for work place exposure to Hepatitis infections in accordance with the Centers for Disease Control and Prevention (CDC) guidelines. Expansions on this Article in local agreements that do not conflict with this minimum standard are permissible.
Immunizations. Immunizations which are determined to be required by the Employer shall be provided free of charge to such employees.
Immunizations. The employing department or program will pay or reimburse employees for immunizations required to perform the functions of their jobs as specified in writing by the department or program.
Immunizations. Where immunizations are required as part of an employee’s employment duties, the cost of such immunizations shall be borne by the Employer.
Immunizations. The District may request proof of immunizations from its employees. Once immunization records have been provided to the District, such records shall be maintained in the employee’s personnel file. The district will assist staff by:
1. Providing opportunity for staff to obtain immunizations that are required by the Public Health Department or any other agency at no cost to the employee, and,
2. Maintaining a database containing immunization records/immunization proof. Any employee who must be excluded from his/her work site as a result of an order by the Public Health Department relating to an outbreak of a vaccine preventable disease, and who has submitted to the District documentation concerning his/her immunization history claiming either a bona fide religious objection to, or medical exemption from, the necessary immunization, shall suffer no loss of pay as a result of the exclusion, provided said employee exercises one of the following options:
1. The employee of his/her own volition decides to utilize his/her accumulated sick leave under the provisions of emergency leave.
2. The employee engages in alternative educational work activities, if agreed to and as identified by the District during the period the employee would have otherwise been performing his/her regular work functions.
3. The employee engages in alternative educational work activities, if agreed to and as identified by the District at some time acceptable to the District and the employee other than during the period the employee would have otherwise been performing his/her regular work functions.
4. Some combination of 1) and 2), above, or 1) and 3) above that is acceptable to the employee and the District.
Immunizations. The MCO must educate Providers on the Immunization Standard Requirements set forth in Chapter 161, Health and Safety Code; the standards in the Advisory Committee on Immunization Practices (ACIP) Immunization Schedule; the AAP Periodicity Schedule for CHIP Members; and the ACIP Immunization Schedule for Medicaid Members. The MCO must educate Providers that Medicaid Members birth through age 20 must be immunized during the Texas Health Steps checkup according to the ACIP routine immunization schedule. The MCO shall also educate Providers that the screening provider is responsible for administration of the immunization and should not refer children to Local Health Departments to receive immunizations. The MCO must educate Providers about, and require Providers to comply with, the requirements of Chapter 161, Health and Safety Code, relating to the Texas Immunization Registry (ImmTrac), to include parental consent on the Vaccine Information Statement. The MCO must notify Medicaid and CHIP Providers that they may enroll, as applicable, as Texas Vaccines for Children Providers. In addition, the MCO must work with HHSC and Providers to improve the reporting of immunizations to the statewide ImmTrac Registry.
Immunizations a) The parties to this Agreement shall encourage Resident Physicians to be appropriately vaccinated in accordance with the recommendations of the Alberta Office of Public Health, including the annual influenza vaccination.
b) The Resident Physician shall be provided free of charge full vaccination services for hepatitis, rubella, influenza and rubella serology and mantoux testing for tuberculosis. Any other immunizations or titre required as a result of or related to the Resident Physician’s duties shall also be provided at no cost.
c) Resident Physicians shall be fit-tested for, and supplied with at no cost to the Resident Physician, N95 masks or other respiratory equipment as required by the Training Facility based upon the Resident Physician assignment.
Immunizations. Charges incurred in connection with immunizations in accordance with the guidelines provided by the United States Preventive Services Task Force. See Exhibit , 'Schedule of Covered Immunizations.'
