Group Life Insurance24. 4 Group Life Insurance Regular employees, who have completed the probationary period shall be provided with a Group Life Insurance Plan. The Plan shall provide ten thousand dollars ($10,000) coverage, until the age of sixty-five (65), and shall include accidental death and dismemberment coverage. After the age of sixty-five (65), the amount of coverage shall decrease to five thousand dollars ($5,000) until the age of seventy (70), at which time group insurance coverage will cease. The Employer shall pay one hundred percent (100%) of the monthly premiums.

  • Key Person Life Insurance The Executive acknowledges that the Company may wish to purchase insurance on the life of the Executive, the proceeds of which would be payable to the Company, at the Company’s expense. The Executive hereby consents to such insurance and agrees to submit to any medical examination and release of medical records required to obtain such insurance.
