ELIGIBLE SERVICE AND FARE VALIDATION. Standard SmartPasses are valid fare for all VTA-operated fixed route Bus and Light Rail services through the Agreement period. A Standard SmartPass is a valid fare on VTA Local Bus and Light Rail services only; premium fare and special event service are excluded from the SmartPass program. Access to VTA Express Bus service is available through the Express SmartPass option. For use on VTA Bus service, Clipper® SmartPass cards will be tagged on the Clipper® reader mounted on the bus upon boarding the bus. For use on VTA Light Rail, SmartPass holders are required to tag their Clipper® SmartPass cards on a Clipper® card reader on the station platform before boarding. SmartPass holders are subject to all VTA fare inspection regulations. The Standard SmartPass is not honored on Highway 17 Express, Dumbarton Express, Caltrain, Monterey-Salinas Transit, Altamont Commuter Express (ACE), or other services not directly operated by VTA. The optional Express SmartPass is available at an additional cost and is also honored as valid fare on VTA Express Bus service, Highway 17 Express, Dumbarton Express, and Monterey-Salinas Transit (MST) service as follows: • On the Dumbarton Express, participants from Institutions with an Express SmartPass Agreement can use Clipper® SmartPass cards the same as they would on VTA-operated service. The Express SmartPass is honored for full fare. • For Express SmartPass participants using the Highway 17 Express or MST services, VTA shall provide annual holographic “E” stickers to be affixed to employee photo ID badges. Participants shall present the badge and sticker to the Highway 17 Express or MST coach operators upon boarding. The sticker together with the badge will be honored for full fare. The Clipper® SmartPass card does not have to be presented. The E sticker is not honored for fare on VTA Bus or Light Rail services, therefore, if a rider needs to use both the Highway 17 Express (or MST) services and VTA services, the rider will need to carry both the Clipper® SmartPass card and the employee photo ID with E Sticker. Employer SmartPass Agreements also provide credit equal to the VTA Adult Base Fare towards a One-Way Trip on VTA’s complementary ADA paratransit service. No credit will be provided for any other type of paratransit trip, including premium services or service charges. For colleges/universities, see section 24 – 30.

  • Outpatient emergency and urgicenter services within the service area The emergency room copay applies to all outpatient emergency visits that do not result in hospital admission within twenty-four (24) hours. The urgicenter copay is the same as the primary care clinic office visit copay.

  • AIN Selective Carrier Routing for Operator Services, Directory Assistance and Repair Centers 4.3.1 BellSouth will provide AIN Selective Carrier Routing at the request of <<customer_name>>. AIN Selective Carrier Routing will provide <<customer_name>> with the capability of routing operator calls, 0+ and 0- and 0+ NPA (LNPA) 555-1212 directory assistance, 1+411 directory assistance and 611 repair center calls to pre-selected destinations.

  • Training Assistance (a) Employees shall be reimbursed for one hundred percent (100%) of the tuition for job-related courses approved by the Employer.

  • Subcontracting for Medicaid Services Notwithstanding any permitted subcontracting of services to be performed under this Agreement, Party shall remain responsible for ensuring that this Agreement is fully performed according to its terms, that subcontractor remains in compliance with the terms hereof, and that subcontractor complies with all state and federal laws and regulations relating to the Medicaid program in Vermont. Subcontracts, and any service provider agreements entered into by Party in connection with the performance of this Agreement, must clearly specify in writing the responsibilities of the subcontractor or other service provider and Party must retain the authority to revoke its subcontract or service provider agreement or to impose other sanctions if the performance of the subcontractor or service provider is inadequate or if its performance deviates from any requirement of this Agreement. Party shall make available on request all contracts, subcontracts and service provider agreements between the Party, subcontractors and other service providers to the Agency of Human Services and any of its departments as well as to the Center for Medicare and Medicaid Services.

  • Preventive Care and Early Detection Services This plan covers, early detection services, preventive care services, and immunizations or vaccinations in accordance with state and federal law, including the Affordable Care Act (ACA), as set forth below and in accordance with the guidelines of the following resources: • services that have an A or B rating in the current recommendations of the U.S. Preventative Services Task Force (USPSTF); • immunizations recommended by the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention; • preventive care and screenings for infants, children, and adolescents as outlined in the comprehensive guidelines supported by the Health Resources and Services Administration (HRSA); or • preventive care and screenings for women as outlined in the comprehensive guidelines as supported by HRSA. Covered early detection services, preventive care services and adult and pediatric immunizations or vaccinations are based on the most currently available guidelines and are subject to change. The amount you pay for preventive services will be different from the amount you pay for diagnostic procedures and non-preventive services. See the Summary of Medical Benefits and the Summary of Pharmacy Benefits for more information about the amount you pay. Preventive Office Visits This plan covers the following preventive office visits. • Annual preventive visit - one (1) routine physical examination per plan year per member age 36 months and older; • Pediatric preventive office and clinic visits from birth to 35 months - 11 visits; • Well Woman annual preventive visit - one (1) routine gynecological examination per plan year per female member.

  • Directory Assistance Service 8.3.1 Directory Assistance Service provides local end user telephone number listings with the option to complete the call at the caller's direction separate and distinct from local switching.

  • Directory Assistance Service Updates 8.3.3.1 BellSouth shall update end user listings changes daily. These changes include:

  • Peer Assistance and Review (PAR) Consulting Teachers (CT) will be assigned to all new teachers with no prior teaching experience and tenured teachers rated ineffective on the qualitative measures at the end of the previous school year and recommended by the PAR Panel.   • DEVELOPMENT OR ASSISTANCE IN DEVELOPMENT OF SPECIFICATIONS REQUIREMENTS/ STATEMENTS OF WORK Firms and/or individuals that assisted in the development or drafting of the specifications, requirements, statements of work, or solicitation documents contained herein are excluded from competing for this solicitation. This shall not be applicable to firms and/or individuals providing responses to a publicly posted Request for Information (RFI) associated with a solicitation.

  • Post-Award Small Business Program Re Representation If applicable, The Contractor shall report timely and accurately their small business program re-representation and update XXX.xxx.
