Common use of Education through Care Coordinators Clause in Contracts

Education through Care Coordinators. The Contractor’s Care Coordinators will attempt to contact all Enrollees who frequently use or recently visited an emergency room to determine whether the Enrollees are experiencing barriers to primary and preventative care, to help resolve those barriers, if any, and to educate Enrollees on the appropriate use of emergency room services and the Enrollees’ medical homes.
