The remittance statement shall be documented by location containing a dues and initiation report which will be provided in the form of e-mail (▇▇▇▇▇@▇▇▇▇▇▇▇.▇▇▇) or on a computer diskette as well as a hard copy of the dues report being attached to the remittance cheque. The information provided shall be on a standard spreadsheet in Excel, Quattro Pro, Lotus or other software program acceptable and adaptable to the Union.
