Any such form completed by the Recipient or Contractor for this transaction shall be incorporated into this Grant or Contract by reference. Recipient or Contractor accepts the responsibility to ensure that that it does not enter into either (a) a subgrant or (b) a contract with a value of $25,000 or more, with an individual or entity which is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participating in this Grant by any Federal department or agency.
