DEBARMENT AND SUSPENSION CLAUSE. A. This contract is a covered transaction for purposes of 2 C.F.R. pt. 180 and 2 C.F.R. pt. 3000. As such the contractor is required to verify that none of the contractor, its principals (defined at 2 C.F.R. § 180.995), or its affiliates (defined at 2 C.F.R. § 180.905) are excluded (defined at 2 C.F.R. § 180.940) or disqualified (defined at 2 C.F.R. § 180.935).

DEBARMENT AND SUSPENSION CLAUSE. Insert if using Federal Funds $25,000 and over. The Recipient or Contractor has certified that it is not presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participating in this transaction by any Federal department or agency. If requested by ODFW, the Contractor shall complete a Certification Regarding Debarment, Suspension, Ineligibility, and Voluntary Exclusion form certifying this information. Any such form completed by the Recipient or Contractor for this transaction shall be incorporated into this Grant or Contract by reference. Recipient or Contractor accepts the responsibility to ensure that that it does not enter into either (a) a subgrant or (b) a contract with a value of $25,000 or more, with an individual or entity which is presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participating in this Grant by any Federal department or agency. Recipient or Contractor may satisfy this requirement by including this clause in any covered lower-tier transaction by obtaining a certificate (See Exhibit D of Sample Contract) certifying such from the subrecipient or contractor or by checking xxx.xxxx.xxx.
