Continuation of Health Coverage and Other Job Benefit Plans. 1. An employee taking leave will continue to participate in any group health care benefits plan under the same terms and conditions, including any necessary co- payments, in which the employee was enrolled prior to the first day of the leave. Co-payments are due monthly. Failure to pay the premium within thirty (30) days of the due date may result in termination of benefits coverage.
