Closing the Loop. Provider shall connect families to necessary supports in the community to gradually integrate the family into healthy activities prior to the closure of the services with the Provider. Provider staff shall work in tandem to ensure the family has been connected and linked to these supports before closure of services. All referral efforts and linkages should be documented appropriately as well as identifying and resolving barriers prior to closure.
