California Public Health Assessment Model (CPHAM. The California Public Health Assessment Model (C-XXXX) was developed by Urban Design 4 Health (UD4H) for planning applications in the State of California (SCAG, 2016). C-XXXX was developed with a variety of survey data in California, including land use, socio-demographic, travel behavior, physical activity, and health conditions. These data were used to derive equations that predict health outcomes based on health-related behaviors and built- environment characteristics of the study area (SCAG, 2016). The relationships among these different variables and health outcomes are depicted in Figure 5. Source: SCAR (2016) Figure 5 The C-XXXX conceptual model. The modeling process of C-XXXX consists of two sets of equations. The first set predicts the amount of physical activities that are likely to be induced by specific demographic and land use variables. The second set then relates the predicted physical activity levels to health outcomes such as body mass index, diabetes, high blood pressure, and cardiovascular disease The first set of equations were derived from California Household Travel Survey and the second set California Health Interview Survey (SCAG, 2016). Because the model was based entirely on California data, C-XXXX cannot be applied in any other states in the United States without re- estimating all predictive equations again.

  • COUNTY’S QUALITY ASSURANCE PLAN The County or its agent will evaluate the Contractor’s performance under this Contract on not less than an annual basis. Such evaluation will include assessing the Contractor’s compliance with all Contract terms and conditions and performance standards. Contractor deficiencies which the County determines are severe or continuing and that may place performance of the Contract in jeopardy if not corrected will be reported to the Board of Supervisors. The report will include improvement/corrective action measures taken by the County and the Contractor. If improvement does not occur consistent with the corrective action measures, the County may terminate this Contract or impose other penalties as specified in this Contract.

Any additional assistance shall be mutually agreed between the Parties.

  • Health and Safety Plan 5. Xxxxxx shall prepare and submit under separate cover from the Work Plan, a Health and Safety Plan consistent with Occupational Safety and Health Administration regulations. The Health and Safety Plan shall be submitted to the Department in the form of one electronic copy on compact disk (in .pdf format). Xxxxxx agrees that the Health and Safety Plan is submitted to the Department only for informational purposes. The Department expressly disclaims any liability that may result from implementation of the Health and Safety Plan by Xxxxxx. PUBLIC PARTICIPATION

  • Public Works Installation work which is considered public works is excluded from purchase under this Contract. Historically, the New York State Bureau of Public Works has maintained that installation, maintenance and repair of equipment attached to any wall, ceiling or floor or affixed by hard wiring or plumbing is public work. In contrast, installation of a piece of equipment which is portable or a “plug-in” free-standing unit would not be considered public work. Thus, this Contract does not authorize installation where the equipment becomes a permanent part of the building structure, or is otherwise incorporated into the fabric of the building (e.g., installation on a wall, ceiling or floor in a fixed location, or affixed by hard-wiring or plumbing). See Xxxxxxxx X, §00, Prevailing Wage Rates - Public Works and Building Services Contracts.   • COMPLIANCE BY CONTRACTOR WITH LAWS AND REGULATIONS/ EQUAL EMPLOYMENT OPPORTUNITY 6.1 In connection with the performance of the Services, the Contractor shall comply with all applicable statutes, laws, regulations, and orders of federal, state, county or municipal authorities which impose any obligation or duty upon the Contractor, including, but not limited to, civil rights and equal employment opportunity laws. In addition, if this Agreement is funded in any part by monies of the United States, the Contractor shall comply with all federal executive orders, rules, regulations and statutes, and with any rules, regulations and guidelines as the State or the United States issue to implement these regulations. The Contractor shall also comply with all applicable intellectual property laws.
