Barcodes. i. The samples that are collected and shipped to Sema4 by the State will be labeled with barcodes that have been printed and provided by Sema4 to the State, and then distributed by the State to the providers, employers, or other organizations for which Sema4 will process tests at the direction of the State; provided that the parties may mutually agree to and put into place other or additional workflows with regard to the barcodes used in connection with this testing. ii. The State will ensure that such providers, employers, or other organizations will enter the Sema4 barcode into Sema4’s Provider/Employer Portal Onboarding (as set forth in Appendix I attached hereto). iii. Such barcodes will be 12 characters (e.g., CT9999999999) that meet the following specifications: 1. CT as the prefix, 2. followed by 10 numerical digits,
