Application and Procedures a. A plan, in writing, must be submitted to the Superintendent by April 1 of the school year preceding the school year for which the sabbatical is requested.
b. The plan shall then be reviewed by the evaluation committee and they shall make recommendations to the Superintendent. This evaluation committee shall consist of four (4) elected tenure teachers (1 elementary, 1 intermediate, 1 middle school, 1 high school) and four administrative representatives.
c. The Superintendent reviews the plan for possible recommendation to the Board.
d. Board action.
e. Notification of applicant, within sixty (60) days of submitting the plan, of approval or rejection.
f. If approved, the applicant must then sign a written agreement stipulating that they will return to the service of the school district after the expiration of the leave for three years. For less service after return to the district, repayment will be prorated.
Application and Procedures. A. An eligible full-time professional staff member and/or the employee’s non- bargaining unit Supervisor may submit an application for Performance-Based Promotion:

1. Current job description 2. A self-assessment statement, no more than three pages, which illustrates how the candidate meets the criteria for promotion as described above.

2. The teacher must file an application on or before March lst to enable the administration to recommend and consider such a leave.
3. The Superintendent shall inform the applicant in written form by May lst that the request has been granted or rejected with the provision of the leave, if granted. In return, the teacher shall notify the Superintendent by June lst if he or she will accept the leave of absence. The written form should include the following:
A. Name of applicant.
B. Beginning and ending dates of said leave of absence.
C. Salary step of applicant upon return to system.
D. Position of applicant upon returning to the school system.
4. The teacher on leave of absence shall receive all rights of tenure and be permitted to remain a part of the group health insurance plan then in effect in the school system if the School District is not restricted by the insurance company from doing so.
