360-Degree Input. 8.3.4.1 A minimum of three sources of 360-degree input is required. The primary evaluator is responsible for sending the 360-degree input form to customers or peers, having the information returned directly to the evaluator, and keeping the information confidential. The primary evaluator shall summarize the 360-degree input and incorporate the aggregate summary information into the classified employee’s evaluation.
