The Plan provides coverage for Basic Services, Endodontics and Periodontics, Restorative Dental and Surgical Procedures and Prosthodontic Services including dentures (Types A, B, C) and the ODA schedule for the immediately preceding year, subject to the eligibility requirements and terms and conditions of the Plan.
Endodontics is the treatment of dental disease of the tooth pulp and includes: • Therapeutic pulpotomy and pulpal debridement, when performed as a final endodontic procedure.
Endodontics, periodontal surgery and restorative treatment are excluded.
Class II—Basic Restorations, Endodontics, Periodontics, Prosthodontic Maintenance and Oral Surgery—to include fillings, root canal treatment, routine extractions, denture adjustments, occlusal biteguards, and periodontal scaling and root planning.
The plan shall reimburse a claimant 100% of the cost of the insured services of Basic Services and 75% of the cost of Endodontics and Periodontal based on the current O.D.A. rate schedule minus one year.
In addition to the "Basic" services listed, all eligible employees and eligible dependents shall be entitled to receive 50% of eligible charges for the "Major" dental services listed below: Endodontics: - the usual procedures required for pulpal therapy and root canal filling.
Endodontics includes root canal therapy, pulp capping procedures, apexification and periapical procedures associated with root canal treatment.
Effective March 05/2003 the Company will add coverage for Endodontics.
