Attributed Life definition

Attributed Life means an individual that receives healthcare benefits from a Payer in an ACO Program and is attributed to ACO in accordance with the terms of an ACO Program Agreement.

Attributed Life or “Attributed Lives” means a Medicare Beneficiary who is assigned to ACO and that qualifies to be included in the Shared Savings calculation performed under the Next Gen Model.

Examples of Attributed Life in a sentence

  • ACO will, for each Attributed Life as established under the BCBSVT Primary Program Agreement, provide payment as set forth in Attachment 1.

  • Next Generation Participant, and Professionals shall make the records available to and communicate as appropriate with each Professional treating the Attributed Life, as needed, for the purpose of facilitating the delivery of appropriate Health Care Services to each Attributed Life and carrying out the purposes and provisions of this Agreement.

  • For each Non-Risk Cohort BCBSVT Attributed Life, OneCare shall pay the Attributing Participant a Primary Care Support Funding of $3.25 (three dollars and twenty five cents) per Assigned Life per month (PMPM).

  • In cases where there is a claim for Primary Care Services (as defined in Attachment A of the BCBSVT Primary Program) provided to the Attributed Life by Attributing Participant within calendar year 2021, $3.25 (three dollars and twenty five cents) per Assigned life per month (PMPM) will be paid by ACO to Attributing Participant.

  • Next Generation Participant with Attributed Lives agrees to provide each Attributed Life, at their first primary care office visit annually, ACO approved information materials to advise such Attributed Life of data sharing and their ability to decline to allow data to be shared with the ACO.

  • If an Attributed Life declines to allow data sharing, Next Generation Participant agrees to maintain and provide proof to ACO that such Attributed Life(ves) has properly opted-out of the data sharing.

Related to Attributed Life

  • Distributed Property shall have the meaning specified in Section 14.04(c).

  • Attribute means an actual or perceived personal characteristic including without limitation race, color, religion, ancestry, national origin, socioeconomic status, academic status, disability, gender, gender identity, physical appearance, health condition, or sexual orientation;

  • Distributed Generation means generating plant equipment collectively used for generating electricity that is connected, or proposed to be connected, to the Network or a Customer's Installation, but does not include:
