Allocation Form definition

Allocation Form means the Settlement Allocation Form, a true and correct copy of
Allocation Form means the Court-approved Allocation Form, substantially in the

  • If you have an existing permit for multiple locations in different cities, you may indicate your sales at the event on your Schedule “C” Allocation Form that is normally provided with your Sales and Use Tax Return.

  • At the same time, the employer agrees to remit periodically to the investment company(ies) selected by the employee on the current Carrier Allocation Form the sum of such reduction as a premium on the annuity contracts, which are purchased by the employer on behalf of the employee.

  • The Demand and Term Deposits Investment Allocation Form allows the Client to allocate its investment between the Term Deposits and/or Demand Deposits.

Related to Allocation Form

  • Election Form means the form established from time to time by the Committee that a Participant completes, signs and returns to the Committee to make an election under the Plan.

  • Distribution Election Form means the form established from time to time by the Plan Administrator that the Director completes, signs and returns to the Plan Administrator to designate the time and form of distribution.

  • Deferral Election Form means each form established from time to time by the Administrator that the Executive completes, signs and returns to the Administrator to designate the amount of Deferrals.

  • Beneficiary Designation Form means the form established from time to time by the Plan Administrator that the Executive completes, signs, and returns to the Plan Administrator to designate one or more Beneficiaries.

  • Application Form means any application form to be completed by subscribers for Shares as prescribed by the Company from time to time.
