Common use of Treatment/Recovery Plan Clause in Contracts

Treatment/Recovery Plan. CONTRACTOR shall develop an individualized treatment/ 13 recovery plan with each Participant within fourteen (14) calendar days of admission into the Program 14 which shall be based upon the Participant’s needs identified in the assessment process. Each treatment 15 plan shall include identification of a minimum of three (3) problem areas, including a drug and/or 16 alcohol problem, long and short term individualized goals for addressing the identified needs with action 17 steps, target dates and dates of resolution for each. Every fourteen (14) calendar days, CONTRACTOR 18 shall review with the Participant, and document, in the progress notes, the Participant’s progress on the 19 treatment plan. CONTRACTOR shall update the treatment plan when a change in problem 20 identification, focus of recovery or treatment occurs, or, no later than ninety (90) calendar days after 21 signing the initial treatment plan, and no later than every ninety (90) calendar days thereafter, whichever Title 22. 22 comes first.
