Sample Dues Deduction Form. The Employer agrees to deduct from the wages of any consenting employee-member of the Union, the dues, initiation fees and assessments of the Union, upon presentation of a written deduction authorization from any employee-member of the Union. All monies deducted for such purposes shall be paid promptly to the Union.
