Phase II Services. Phase II Services shall mean and include all of the following services: a. Develop and implement a design coordination framework to facilitate the design of the project by the Design Professional and the design coordination activities of the CM/▇▇ ▇. Attend design coordination meetings between the Design Professional and CM/▇▇ ▇▇. Prepare and distribute minutes of design coordination meetings iii. Advise the Owner and Using Agency as to the systems and components of the Project that should be commissioned. iv. If directed by Owner, develop and coordinate with the Design Professional a building commissioning plan consistent with state and Regent’s guidelines (See Exhibit A.) b. Develop and implement a cost management (cost containment) strategy that will identify and track decisions that have an impact on cost. i. Independently identify the costs associated with each phase of the Project that reflects the program scope ii. Develop and implement a cost model. iii. Develop and implement a cost tracking methodology that will allow all parties to compare projected costs with actual costs in “real time." iv. Include a methodology to inform all parties, including the Design Professional and the CM/GC, if the model projects that expended costs will exceed the planned cost v. Identify three suggested methods to regain balance between scope and cost. vi. As a minimum, cost containment strategies should include the following considerations: Maintenance of program scope; Maintenance of the Project schedule Construction quality assurance and compatibility with Owner/Using Agency’s design standards. Provision of all necessary services and utilities. Keeping the Using Agency fully aware of the progress of the Project. vii. Create a project file that tracks all cost-related decisions and includes the rationale for them; provide the Owner with this record in bound form upon the completion of the Project. c. Represent the interests of the Owner and Using Agency and the user groups during the preparation of schematic drawing, design development documents, and construction documents. i. Monitor and document the Design Professional's program validation process and provide Owner and Using Agency (including user-group representatives) with a copy of the validated program, signed-off on by Using Agency and the Design Professional. ii. Phase II Services. 5.2.2.1 Within fourteen (14) days from the date of the Agent's acceptance of the Phase I Plan of Development, the Developer shall prepare, or cause others to prepare, and submit to the Agent, for the Agent's acceptance, an updated plan of development consisting of, among other things, the Design Development Documents, a revised Development Schedule, and a revised Budget (the "Phase II Plan of Development"). Upon receipt of a complete Phase II Plan of Development from the Developer, the Agent shall have fourteen (14) days to review same (the "Phase II Review Period"). On or before the expiration of the Phase II Review Period, the Agent shall either: (i) accept the Phase II Plan of Development by transmitting a written notice of acceptance to the Developer, or (ii) transmit, in writing, its review comments to the Developer. Upon the receipt of such review comments by the Developer, but in no event later than fourteen (14) days from the date of receipt by the Developer of such review comments, the Developer shall incorporate such review comments into the Phase II Plan of Development or shall notify the Agent, in writing, that such review comments, if it is the case, cannot be incorporated into the Phase II Plan of Development because such review comments, if incorporated into the Phase II Plan of Development, would have a material and adverse impact upon the Project (a "Phase II Issue"). In the event that the Developer incorporates such review comments into the Phase II Plan of Development, the Developer shall resubmit the Phase II Plan of Development to the Agent and the above-described process of the Agent's review and the Developer's revisions will continue until the Agent accepts the Phase II Plan of Development; provided, however, the Agent, in connection with its review of any comments, may not include any new review comments unrelated to those that were previously provided in its prior comments to the Phase II Plan of Development. Additionally, if the Agent fails to provide its review comments within the appropriate Phase II Review Period, the Phase II Plan of Development shall be deemed accepted upon the expiration of most current Phase II Review Period, except as to any unaddressed review comments remaining from any prior Phase II Review Period. In the event of the occurrence of a Phase II Issue, the Agent shall be entitled to avail itself of the dispute resolution procedure set forth in this Agreement to resolve such Phase II Issue. Phase II Services. Phase II services will be negotiated after identification of the funding source for Phase II and selection of the project delivery method by the County. Any and/or all payments made under this Agreement shall be paid by check, payable to the order of the CONSULTANT and be mailed or delivered to CONSULTANT at: Name: Address: City/State/Zip: CONSULTANT may request that COUNTY mail the check to CONSULTANT to such other address as CONSULTANT may from time to time designate to COUNTY. Such request must be made in writing in accordance with the procedures as outlined under Section “NOTICES”.
Phase II Services. Within a commercially reasonable period of time after it receives a Site Acceptance Notice pertaining to a Potential Site that resulted from Conestoga's Batch Search Ring Submission, and upon receipt of the Testing Data from Conestoga, Mountain Union shall obtain permission from the owner of the Potential Site for drive testing and/or site visits ("Initial Site Tests") and provide Conestoga with such other information and documentation as may be reasonably requested by Conestoga with respect to the Initial Site Tests. For Site Acceptance Notices received by Conestoga on an individual basis, Mountain Union shall, within ten (10) business days after it receives the Site Acceptance Notice and the relevant Testing Data from Conestoga, obtain the requisite authorization from the Potential Site Owner for the Initial Site Test pertaining thereto. As used herein, "Testing Data" means: (i) requested dates and times for the performance of the Initial Site Tests; (ii) the proposed scope of the testing (as determined by Conestoga in its commercially reasonable discretion); (iii) the power requirements applicable to the Initial Site Tests; (iv) the execution of all documents required by the Potential Site owner with respect to the Initial Site Tests; and (v) making the appropriate personnel available to participate in the Initial Site Tests with Mountain Union. Mountain Union shall, in conjunction with Conestoga personnel, conduct the Initial Site Tests on a schedule to be agreed upon by the Potential Site Owner, Conestoga, and Mountain Union.
Phase II Services. If the District decides to proceed with Phase II, payment of all Brokerage Fees associated with the Phase II Services, shall be invoiced and paid after completion of the Phase II Services.

This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.
