Measurement Methodology. See methodology provided for performance measure below. Outcome Measure #1 75% of individuals engaged in services via street/encampment and library outreach, respectively, will complete a VI-SPDAT to identify individual’s vulnerability and service prioritization. Outcome will be measured via HMIS and internal tracking system that will document all individual engagements and those that have been assessed via the VI-SPDAT.
