Laboratory Testing Services. The soil/rock samples recovered from the field exploration program will be transported to our Miami laboratory for better classification testing and to determine the pertinent engineering properties for foundation design and site preparation recommendations. Any laboratory testing that may be performed will be limited to soil classification testing. At the completion of the field and laboratory testing services, our Geotechnical Project Engineer will prepare a data report under the direction of a registered Professional Engineer who specializes in geotechnical engineering consulting. Laboratory Testing Services. The Contractor shall: 4.14.1 Use laboratory testing sites that have either a Clinical Laboratory Improvement Amendments (CLIA) Certificate of Waiver or a Certificate of Registration along with a CLIA identification number. 4.14.2 Verify that laboratories satisfy all requirements in, 42 CFR 493, Subpart A, General Provisions. 4.14.3 Cover laboratory services for diagnostic, screening and monitoring purposes when ordered by the member’s PCP, other attending physician or dentist, and provided by a CLIA approved free-standing laboratory or hospital laboratory, clinic, physician office or other health care facility laboratory. 4.14.4 Require all clinical laboratories to provide verification of CLIA Licensure or Certificate of Waiver during the provider registration process. Laboratory Testing Services. Applicant has identified a need to conduct certain tests that: (a) require use of a unique or special University facility (the “Laboratory”) that either does not exist elsewhere or is not readily accessible; and (b) involve established, pre-existing methods of a primarily technical nature; and (c) do not constitute original, creative research (clauses (a) – (c) are collectively referred in this Agreement as “Laboratory Tests”). University, through its faculty and staff, will use reasonable efforts to perform the Laboratory Tests identified in one or more SOWs (defined below) which identify the specific testing methods and unique University equipment/facilities to be used and the objectives to be achieved (“Services”), in accordance with and subject to this Agreement. The Services shall be described in one (1) or more properly authorized and executed Statement of Work (each, an “SOW”), which is set forth in substantial form and substance in Exhibit A to this Agreement (“Exhibit A”) and is attached hereto and incorporated in this Agreement by this reference. If a conflict exists between the terms of this Agreement and any SOW, the terms of this Agreement shall govern. University shall not be legally obligated to perform any Services (including, the Laboratory Tests) not set forth in the applicable SOW.
Laboratory Testing Services. Contractor shall provide the laboratory services to conduct PCR and/or serological testing of all test samples collected by the Contractor’s mobile/community testing teams and the Contractor’s CFTTs. At LDH’s request, Contractor may further provide laboratory services to conduct testing of test samples not collected by Contractor’s testing teams but only to the extent that Contractor determines that Contractor’s laboratory has capacity to do so and Contractor so notifies LDH.
