Common use of Individual Coverage Clause in Contracts

Individual Coverage. Effective February 1, 2013, the School District shall contribute a sum not to exceed $460 per month for each eligible employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization plan. Effective December 1, 2013, the School District shall contribute a sum not to exceed $465 per month for each eligible employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization plan. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.

Individual Coverage. Effective February July 1, 2013, 2018 the School District shall contribute a sum an amount not to exceed $460 765 per month for each eligible employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization plan. Effective December Effective, January 1, 20132019 the total monthly contribution shall increase, but shall not exceed $780 per month. Effective January 1, 2020, the School District total monthly contribution shall contribute a sum increase but shall not to exceed $465 795 per month for each eligible employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization planmonth. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.

Effective February July 1, 20132014, the School District school district shall contribute a sum not to exceed $460 565 per month for each eligible employee employed by the School District school district who qualifies for and is enrolled in the School District school district group health and hospitalization plan. Effective December January 1, 20132015, the School District school district shall contribute a sum not to exceed $465 635 per month for each eligible employee employed by the School District school district who qualifies for and is enrolled in the School District school district group health and hospitalization plan. The cost of the premium not contributed by the School District school district shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.

Effective December January 1, 2013, the School District 2017,the school district shall contribute a sum not to exceed $465 700 per month for each eligible employee employed by the School District school district who qualifies for and is enrolled in the School District school district group health and hospitalization plan. The cost of the premium not contributed by the School District school district shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.

Effective December January 1, 20132022, the School District shall contribute a sum not to exceed $465 824 per month for each eligible employee employed by the School District who qualifies for and is enrolled in the School District group health and hospitalization plan. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.

Effective December January 1, 20132025, the School District total monthly contribution shall contribute a sum increase but shall not to exceed $465 804 per month for each eligible employee employed by month. Effective January 1, 2026, the School District who qualifies for and is enrolled in the School District group health and hospitalization planschool district shall no longer offer this plan to employees. The cost of the premium not contributed by the School District shall be borne by the employee and paid by payroll deduction. This subdivision shall not apply to those eligible employees who select coverage under the high deductible health plan described in Subdivision 3.
