Common use of INDIVIDUAL CASE MANAGEMENT PROGRAM Clause in Contracts

INDIVIDUAL CASE MANAGEMENT PROGRAM. The INSURER will establish a program to identify and manage cases that involve high health care costs, as described in the INSURER's Proposal. The INSURER shall submit a report to the ADMINISTRATION on a quarterly basis of all findings in the Individual Case Management Program. The ADMINISTRATION may review and/or audit the program findings and reports at any time.
