This format may be modified as necessary to effectively communicate information on the activities funded by this Grant Agreement, and includes the following: The Executive Summary consists of a maximum of twenty (20) pages summarizing information for the grant. • Summary of the work competed. • Brief comparison of work proposed in the original Water Quality, Supply, and Infrastructure Improvement Act of 2014 Sustainable Groundwater Planning Grant application and actual work done. • Identify remaining work and mechanism for their implementation. • A summary of final funds disbursement for the project. • A final schedule showing actual progress duration versus planned progress. EXHIBIT G REQUIREMENTS FOR STATEWIDE MONITORING AND DATA SUBMITTAL Surface water quality monitoring data shall be prepared for submission to the California Environmental Data Exchange Network (CEDEN). The CEDEN data templates are available on the CEDEN website. Inclusion of additional data elements described on the data templates is desirable. Data ready for submission should be uploaded to your CEDEN Regional Data Center via the CEDEN website. CEDEN website: ▇▇▇▇://▇▇▇.▇▇▇▇▇.▇▇▇. Indicate personnel, hours, rates, type of profession and reason for consultant, i.e., design, CEQA work, etc. o Evaluation cost information, shown by material, equipment, labor costs, and any change orders o Any other incurred cost detail o A statement verifying separate accounting of funding disbursements • Summary of project cost including the projectfollowing items: o Accounting of the cost of project expenditure; o Include all internal and external costs not previously disclosed; and

e) Guidelines, policy, and procedures on State funded Program/Project
3. Audit reports of the Agency internal control structure and/or financial statements within the last two years.
4. Prior audit reports on the State funded Program/Project. Where applicableState Funding:
