Date of Deposit Security Deposit of $ ONKAD-ONKA Deposit 30-000-286.000 Grand Total of Fees MIKO-Rental Fees 30-252-509.000 Additional Hours of usage HOLD HARMLESS AGREEMENT ALCOHOLIC BEVERAGES PUBLIC FACILITIES SITE: DATE: In consideration for being allowed to make alcoholic beverages available to guests upon the above described public facilities (“Facility”) and date, I acknowledge that I either have or will immediately inspect the premises for its safety and use for the availability of alcoholic beverages, and that my entry upon the area constitutes an acknowledgment that I have inspected the public area for the express purpose of the use of alcoholic beverages and find it reasonably suited for that purpose. I agree that if at any time I find that use to be unsafe, I will immediately notify the Sugar Creek Police Department and that I will immediately leave the public area. I agree that use of the premises, including the use of alcohol therein, shall be in compliance with all applicable local, state and federal laws and regulations, and that I take responsibility for said compliance. I agree that if at any time I find or believe that any laws or regulations may be being violated, I will immediately notify the Sugar Creek Police Department and that I will immediately leave the public area. The undersigned shall be responsible for any damage to the above-described public facility and injury to any person(s) attending the event, whether or not caused by the acts or omissions of the City of Sugar Creek, its agents, employees (“City”), guests or invitees. The undersigned agrees to indemnify and hold harmless the City from all claims, damages, suits or liabilities that may arise or occur from the undersigned’s rental or use of the Facility. The undersigned agrees to indemnify, defend and hold harmless the City for any liability, cost or claims for personal injury, property damage or any violation of local, state or federal law arising out of or in any way in connection with or caused by the negligent, willful or intentional act or omission to act of the undersigned or the undersigned’s guests or invitees. The undersigned takes full responsibility for and holds the City harmless for any action that results in property damage or injury to themselves, their guests or invitees, or any others any others related in any way to the use of alcohol, medications, or any other substances. The City shall not be responsible for its inability to perform the entire or any portion of this agreement if it is prevented from doing so by circumstances beyond its control (e.g. – weather, acts of nature, etc.). In the event of such or any circumstances that prevent the City from performing either the entire or any portion of this agreement, the City shall be limited to returning only the amounts previously deposited by the undersigned. I expressly acknowledge and agree that the consumption of alcoholic liquor may be very dangerous and involve the risk of serious injury and/or death and/or property damage. I further expressly agree that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the laws of the State of Missouri in which the consumption of alcoholic liquor upon a public facility occurs and that if any portion thereof is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect. I have read and voluntarily sign this release and waiver of liability and indemnity agreement, and further represent that no oral representations, statements or inducements apart from the foregoing written agreement have been made. A $300 security deposit check is mandatory and must be paid 30-days prior to your rental date. (Events held by Township-based nonprofit organizations, youth groups, and the negligentMontague Township Seniors group are exempted from the security deposit requirement). Your security deposit will be returned the week following your rental, willful or intentional act or omission to act providing there is no violation of the undersigned or the undersigned’s guests or inviteesthese rules and regulations. The undersigned takes full responsibility for and holds the City harmless for any action that results in property damage rental payment will be due on or injury before 30 DAYS prior to themselves, their guests or invitees, or any others any others related in any way your event! The key to the Community Center can be obtained from the Community Center Caretaker - Xxxx Xxxxxxxxxxx – 000-000-0000 Any use of open flames is NOT permitted on township property including candles, hot plates, and sternos Smoking is strictly prohibited inside of the facility and on the grounds. The Township is not responsible for loss or theft of personal items from the building or vehicles. Alcoholic beverages are NOT permitted inside of the facility, outside the building, or anywhere on the grounds. Decorations are acceptable but no tacks, nails, staples, glitter, or confetti. PAINT SAFE TAPE ONLY All decorations and tape must be removed at the time of departure. Renter must remove all equipment/supplies from building immediately following event and return the key to the drop off box located behind the municipal building (black drop off box to the left of the rear door) Renter must clean all countertops and tables. Chairs must be turned over on top of tables. Room should be left as they were found. Any accidents or emergencies should be reported to the NJ State Police (000)-000-0000 All trash must be placed in receptacles. All recyclables consisting of glass, plastic and cans must be placed in cans marked for Recycling. It is the renters responsibility to take all garbage and recycling from facility. All lights must be turned off, and all the doors and windows must be secured upon departure. Thermostats must be returned to their original temperature setting. *Any violation of these rules and regulations or damage to the facility will result in forfeiture of security deposit.
