Fellow's Responsibilities. The Fellow agrees to the following: 10.1 Obey and adhere to the policies, procedures, rules, bylaws, and the regulations (collectively the "policies") of MAHEC as noted in the Resident Physician Manual and any program specific manuals. Likewise, the Fellow shall obey and adhere to the corresponding policies, procedures, rules, bylaws, and regulations of all of the affiliating institutions, practices, and/or facilities to which he or she is assigned. By signing this Agreement, the Fellow acknowledges having received, read, and understood the policies and provisions listed in the Resident Physician Manual. 10.2 Obey and adhere to all applicable state, federal, and local laws, rules, and regulations, MAHEC’s Corporate Compliance Plan, HIPAA policies and procedures as well as the standards required to maintain accreditation by the JCAHO, the ACGME, the RRC, and any other relevant accrediting, certifying, or licensing organizations, and the Compliance and/or HIPAA Plans of all major participating and affiliated institutions and non-hospital settings where didactic and clinical activities occur during the course of the GME Program. 10.3 Perform as a Fellow under appropriate supervision for patients at the affiliating institutions, practices, and/or facilities to be selected by MAHEC faculty, attending or precepting physicians. The Fellow will fulfill the defined responsibilities for each service rotation and postgraduate year. 10.4 Participate fully in the educational and scholarly activities of the GME program, including the performance of scholarly and research activities as assigned by the Program Director and/or as necessary for the completion of applicable graduation requirements. 10.5 Attend all required educational conferences; assume responsibility for teaching and supervision of residents and students in accordance with policies; and participate in committees of the affiliating institutions and MAHEC. 10.6 Use his or her best efforts to provide safe, effective, and compassionate patient care and present at all times a courteous and respectful attitude toward all patients, colleagues, employees and visitors at MAHEC, affiliating institutions, practices, and/or facilities to which the Fellow is assigned. Fellow's Responsibilities. By participating in the Program, the Fellow agrees to commit to the following: • Participation of Fellow in training bootcamp from May 17, 2020 to May 30, 2020. Fellow will be receiving up to $350 during the bootcamp for living stipend and incidentals. To receive full amount, Fellow will need to be present for entirety of bootcamp training; • Engaging in a one-year, full-time job assignment in a substantive position that contributes directly to the Employer’s mission and goals; • Recognizing and acknowledging that he or she is representing IDF, the Program and their Host Employer/Organization, and commit to doing so in a professional manner. This includes punctuality, professional dress, language, and appropriate use of technology; • Notify Hacker Fellows program director if planning to or have been removed from position with Employer; • If Fellow is unemployed, Fellow agrees to actively seek employment with a Michigan-based company during Program commitment; • Fellow is required to write one (1) blog post during the program. Blog post will be published on the Hacker Fellows website and is 1,500 words or less. Fellow will commit to the Program and Host Employer/Organization for a one-year period of time beginning May 17, 2020 and ending June 30, 2021 unless earlier terminated in accordance with the Termination section below. Fellows are at-will employees of the Host Employer/Organization and will be subject to the personnel policies, guidelines and procedures, including evaluation and disciplinary procedures, of the Host Employer/Organization. If the performance of the Fellow is not acceptable according to Host Employer/Organization requirements and guidelines then the Fellow’s employment may be terminated by the Host Employer/Organization. A Fellow’s participation in the Program is also subject to termination at any time. Specifically, a Fellow’s participation in the Program may be terminated under the following conditions and circumstances prior to June 30, 2021: • The private philanthropic foundation grant funds obtained by IDF to launch and support the Program including training and networking activities, are discontinued; • The Fellow is terminated from the Program, by the Program, due to failure or inability to participate in Program events or other required activities on two (2) or more unacceptably excused occasions; • Fellow is mutually terminated from the Program due to personal reason affecting the Fellow’s ability and/or commitment to participa...
Fellow's Responsibilities. Fellow agrees : (A) To provide the Program Director with written notification of any change in status regarding employment eligibility in Article I(C). above (B) To perform faithfully and satisfactorily and to the best of his/her ability during the entire period of this Agreement the customary duties of Fellows including the satisfactory completion of medical records in accordance with NCH’s standards, rules, policies, and Bylaws or those of the clinical setting in which patient care is provided, if outside NCH. (C) To abide by the Medical Staff Bylaws, NCH’s policies and procedures (including but not limited to Administrative, Patient-Family Care, GME, Disaster, Personnel, Infection Control, Mandatory training and health requirements, and Corporate Compliance), the terms of this Agreement, and to understand and agree that infractions thereof are justification for dismissal. (D) To accept the agreed upon stipend, experience and instruction as sole compensation and not accept fees in any form from patients/parents or others. Additionally, Fellow shall not accept loans or funding from other medical groups or corporations without prior approval from the Program Director and the Designated Institutional Official (DIO). (E) That he/she is not under obligation to any other hospital or healthcare provider in any capacity during this period except as may be designated as part of the Program. (F) To abide by all ACGME duty hour requirements and report faithfully and accurately all duty hours in accordance with the procedure established for the Program. To notify the Program Director and/or the Designated Institutional Official immediately of any violations of the duty hours policy. Failure to comply with the duty hours requirements or failure to notify the Program Director and/or the DIO may result in disciplinary action. (G) To secure a National Provider Identification (NPI) number. (H) To notify the NCH Pharmacy of his/her personal Drug Enforcement Agency (DEA) number and to utilize this DEA number during the course of the Program. If Fellow does not have a personal DEA number, he/she must utilize a NCH DEA with identifier suffix until a personal DEA is obtained. NCH’s DEA number with identifier suffix is restricted for use by the Fellow only in the course of carrying out clinical duties within the Program. Fellow's Responsibilities. 1. The Fellow agrees to execute and adhere to the CSTE Fellowships Code of Conduct, attached to and incorporated into this MOU by reference. 2. The Fellow shall submit to CSTE a quarterly Plan of Action which provides updates to the activities from the original Plan of Action in III(1) above. Quarterly reports describe completed activities, accomplishments to date, and any new activities. 3. The Fellow shall complete any onboarding documentation and education required by County, including assurances that Fellow will comply with all applicable Federal, State and local confidentiality and compliance mandates in the performance of the fellowship, including but not limited to HIPAA, 42 CFR Part 2, California Civil Code Sections 56-56.16, and California Welfare and Institutions Code Sections 10850 and 5328. 4. Fellows may seek approval from their host site and CSTE to participate in a short- term course but are not allowed to be full-time students seeking a degree during the AEF fellowship. 5. The Fellow must provide any documentation to CSTE that is required to graduate from the AEF and receive certification, including documentation of completion of all required core activities, submission of a final report to CSTE, and satisfactory performance during the fellowship per the reports of the Fellow’s mentors.
Fellow's Responsibilities. A. Provide the Graduate Medical Education office with all requested credentialing information; incomplete paperwork will delay the start of your training program. B. Comply with Children's National's immunization, health screening, and drug testing requirements. C. Satisfy all requirements for employment by Children's National. D. Comply with each Affiliated Institution requirements and applicable state laws to which fellow is assigned. E. Complete all requirements for promotion to the next level of training. F. Review the requirements for specialty board examination and register to take the Boards during the first year of fellowship. Fellows are not permitted to start training at Children's National until they have obtained a full DC license; or applicable DC training license. M. Perform duties under this Agreement during such hours as the Program Director may direct in accordance with Children's National's Program Duty Hours and Moonlighting Policies, including reporting to the program director or GME Office any duty hour violations or concerns.   • COUNTY’S RESPONSIBILITIES A. A County program liaison will monitor the submission of all correspondence required in this Agreement, including, but not limited to: 1. Quarterly Treatment Reports; 2. Financial reports such as annual budgets, cost allocation plans, and cost reports; 3. Incident reports; 4. Outcome data; 5. Monthly ▇▇▇▇▇ Reports 6. Other requested reports B. A County program liaison may visit Contractor during the contract term. The visits shall be for the purpose of reviewing any aspect of Contractor’s program operations. The visit may include, but is not limited to: 1. Review all pertinent participant records. 2. Conduct appropriate interviews/discussions with participants served by Contractor. 3. Review and monitor all correspondence and reports submitted by Contractor related to Contractor’s services provided under this Agreement. 4. Meet with appropriate program management and operations staff. 5. Conduct site visit(s) to Contractor’s program(s) at least once during the term of the Agreement to review all aspects of program operations. Site visit(s) may include a review of Contractor’s programmatic and fiscal documentation related to required reports on services specified in the Exhibits. a. Provide a written site review report documenting areas of compliance and any necessary corrective action(s) required. 6. A County program liaison may attend an organized activity of a selected component or selected components of Contractor’s program(s) at least once during the contract term. C. AODS will conduct periodic mandatory treatment provider meetings with representatives of all contracted service providers and appropriate staff. D. Provide ongoing technical assistance as needed. E. AODS shall act as intermediary on behalf of each contracted alcohol and drug service provider in the submission of the California Outcomes Measurement System (CalOMS) data submissions to the State of California. I. GENERAL ADMINISTRATIVE REQUIREMENTS A. Attend each of the following meetings: 1. Contractor shall attend periodic mandatory meetings; and 2. Drug and Alcohol Information System for You (DAISY) User Group meeting. 3. Other meetings as required by the County B. Contractor shall acknowledge the San Mateo County Alcohol and Other Drug Services (AODS) and/or the County of San Mateo as a funding source on newly developed promotional materials. C. Subcontracting requirements: 1. Pursuant to paragraph 12 of the body of this Agreement, Contractor may subcontract for provision of services described in this Agreement with written approval of the Director of the Human Services Agency or her designee. If Contractor subcontracts for any services under this Agreement, Contractor will guarantee that any and all subcontractors have and maintain the same level of insurance coverage required of the Contractor under this Agreement. Contractor and County will be listed as additional insured on all applicable insurance of subcontractor.

  • CITY’S RESPONSIBILITIES 2.1. The CITY shall designate in writing a project coordinator to act as the CITY's representative with respect to the services to be rendered under this Agreement (the "Project Coordinator"). The Project Coordinator shall have authority to transmit instructions, receive information, interpret and define the CITY's policies and decisions with respect to the CONTRACTOR's services for the Project. However, the Project Coordinator is not authorized to issue any verbal or written orders or instructions to the CONTRACTOR that would have the effect, or be interpreted to have the effect, of modifying or changing in any way whatever: (a) The scope of services to be provided and performed by the CONTRACTOR; (b) The time the CONTRACTOR is obligated to commence and complete all such services; or (c) The amount of compensation the CITY is obligated or committed to pay the CONTRACTOR. 2.2. The Project Coordinator shall: (a) Review and make appropriate recommendations on all requests submitted by the CONTRACTOR for payment for services and work provided and performed in accordance with this Agreement; (b) Arrange for access to and make all provisions for the CONTRACTOR to enter the Project site to perform the services to be provided by the CONTRACTOR under this Agreement; and (c) Provide notice to the CONTRACTOR of any deficiencies or defects discovered by the CITY with respect to the services to be rendered by the CONTRACTOR hereunder. 2.3. The CONTRACTOR acknowledges that access to the Project Site, to be arranged by the CITY for the CONTRACTOR, may be provided during times that are not the normal business hours of the CONTRACTOR. 3.1. Services to be rendered by the CONTRACTOR shall be commenced subsequent to the execution of this Agreement upon written Notice to Proceed from the CITY for all or any designated portion of the Project and shall be for a period of two (2) years and three (3) month, with the option for two (2) additional one (1) year renewal. Time is of the essence with respect to the performance of this Agreement. 3.2. Should the CONTRACTOR be obstructed or delayed in the prosecution or completion of its services as a result of unforeseeable causes beyond the control of the CONTRACTOR, and not due to its own fault or neglect, including but not restricted to acts of God or of public enemy, acts of government or of the CITY, fires, floods, epidemics, quarantine regulations, strikes or lock-outs, then the CONTRACTOR shall notify the CITY in writing within 5 working days after commencement of such delay, stating the cause or causes thereof, or be deemed to have waived any right which the CONTRACTOR may have had to request a time extension. 3.3. No interruption, interference, inefficiency, suspension or delay in the commencement or progress of the CONTRACTOR's services from any cause whatsoever, including those for which the CITY may be responsible in whole or in part, shall relieve the CONTRACTOR of its duty to perform or give rise to any right to damages or additional compensation from the CITY. The CONTRACTOR's sole remedy against the CITY will be the right to seek an extension of time to its schedule. This paragraph shall expressly apply to claims for early completion, as well as claims based on late completion. [If Applicable-Provided, however, if through no fault or neglect of the CONTRACTOR, the services to be provided hereunder have not been completed within 18 months of the date hereof, the CONTRACTOR's compensation may be equitably adjusted, with respect to those services that have not yet been performed, to reflect the incremental increase in costs experienced by the CONTRACTOR after expiration of said 18 month period.] 3.4. Should the CONTRACTOR fail to commence, provide, perform or complete any of the services to be provided hereunder in a timely and reasonable manner, in addition to any other rights or remedies available to the CITY hereunder, the CITY at its sole discretion and option may withhold any and all payments due and owing to the CONTRACTOR until such time as the CONTRACTOR resumes performance of its obligations hereunder in such a manner so as to reasonably establish to the CITY's satisfaction that the CONTRACTOR's performance is or will shortly be back on schedule.

  • Client’s Responsibilities The Client’s responsibilities include the following: a. Enrolling. The Client must complete the Informed Consent for Collection and Storage (attached as Schedule 1) The Client must complete or facilitate the Gestational Carrier’s and if applicable, Egg Donor’s completion of the Health History Questionnaire and the Informed Consent to Testing of the Maternal Sample provided by ViaCord. For ▇▇▇▇ ▇▇▇▇▇▇▇ collected in New York State, where possible, the Child’s biological father should also complete a Health History Questionnaire. Generally, for Newborn Stem Cells to be used in treatment, the healthcare provider will need information about the Gestational Carrier and if applicable, Egg Donor. The Health History Questionnaire(s) provides much of the required information.Complete and accurate information is critical to Release and use of Newborn Stem Cells. If You do not provide a completed Health History Questionnaire(s) from the Gestational Carrier and if applicable, Egg Donor, VPL may be unable to Release the Newborn Stem Cells. In addition, if any information provided in the ViaCord Services Agreement or any of the Health History Questionnaire(s) is incomplete or incorrect, it is the Client’s responsibility to notify ViaCord and correct that information immediately.

Residents will accept the duties, responsibilities, and rotations assigned by the Program Director and abide by the rules and regulations of the hospitals and clinics to which the resident is assigned to the extent consistent with this Agreement and Providence and Program policies; conduct themselves ethically and professionally in keeping with their position as a physician; and abide by the terms of this Agreement. The Program Director may delegate to a faculty member in the Program any of the authorities, duties and/or responsibilities of the Program Director. References in this Agreement to “Program Director” include a designee, if any, to whom the Program Director has made such a delegation. B. The residency program provides faculty supervision of residents in the Program. This responsibility includes defining Program content, evaluating Program quality and evaluating individual residents’ clinical training and performance. Additionally, Providence and the Program perform a series of administrative and educational functions for the benefit of residents and the participating institutions and clinics. C. Providence, as the sponsoring institution, appoints a Designated Institutional Official (DIO) who, in conjunction with the Graduate Medical Education Committee (“GMEC”), has the responsibility to monitor and ensure compliance with all ACGME Common, Specialty/Subspecialty-Specific Program and Institutional Requirements. Each Program will have two (2) peer-selected resident representatives with full voting privileges on the GMEC. D. The sponsoring and participating institutions provide: educational and clinical opportunities for residents; services and systems to minimize residents’ work that is extraneous to their GME educational goals to include [without limitation] patient support services, laboratory/pathology/radiology services and medical records; funds for resident salaries, funds and/or services for the benefits provided to residents, and funds for administration of the Program. E. Providence and participating institutions will assure the availability of meals; rest and sleeping quarters; and support facilities conducive to the educational process which includes adequate communication resources and technological support. Providence and participating institutions have agreed to provide on-call rooms with available bathroom facilities. There shall be a sufficient number of on-call rooms so that while on-call residents may sleep and have a secured storage area for books and clothing. Providence and participating institutions will assure there are clean and private facilities for lactation that have refrigeration capabilities.
