Eye Examination. Full time and regular part time employees of the Department are eligible for reimbursement for eye examinations according to the following: 1. A full time employee may receive a maximum reimbursement in any calendar year for out of pocket expenses not to exceed $75. 2. A regular part time employee who has been continuously employed at NAED for 12 months or more may receive reimbursement for a percentage of his/her out of pocket costs for eye examinations (up to the maximum provided, below). The reimbursement percentage will be determined by dividing the number of hours for which the employee has been paid during the preceding 12 months by 2080 hours. The employee’s reimbursement will be the lesser of: 1.) the amount of the out of pocket costs for the examinations multiplied by the reimbursement percentage, or 2.) $75 multiplied by the reimbursement percentage. 3. A regular part time employee who has been continuously employed at NAED for less than 12 months may receive reimbursement for a percentage of his/her out of pocket costs for eye examinations (up to the maximum provided, below). The reimbursement percentage will be determined by dividing the number of hours for which the employee has been paid during the preceding 12 months by the number of weeks that the employee has been employed during the preceding 12 months and then dividing the resulting number by forty (40). The employee’s reimbursement will be the lesser of: 1.) the amount of the out of pocket costs for the examinations multiplied by the reimbursement percentage, or 2.) $75 multiplied by the reimbursement percentage.
